Stratford Academy issued the following announcement on Oct. 14.
Dear Parents,
We are pleased to announce Stratford Academy's second Student Flu Vaccine Campaign! On Tuesday, October 19th, we will be offering Flu shots to all Stratford students from Beginners through 12th grade. We have partnered with Dr. W. Jason Woods, LLC, to be able to offer flu shots to every Stratford student who would like to get one.
Flu shots will be offered on October 19th between 7:45 a.m. and 1:30 p.m. in the Nurse's Clinic in the Cantrell Lower School. (For Preschool students, the nurses will take the immunizations to the Preschool clinic during the morning and give the PS students their shots there.). All LS, MS, and US students who return their form will have an opportunity during the appointed time to visit the Clinic for their shot.

If you would like for your child to receive a flu vaccine at school next week, please:
Print, complete, and sign the Consent Form
Return your signed Consent Form to school with a copy of your insurance card (front and back) on Tuesday, October 19th
Email victoria.lee@stratford.org and either your child's homeroom teacher for PS and LS students or susan.jones@stratford.org for MS and US students and let them know you would like for your child to get a flu shot on Tuesday, October 19th. This will help us check for form and remind students to go.
(If you do not have access to a printer at home and would like to participate, please email Kelly Causey at kelly.causey@stratford.org and she will ensure that a paper form is sent home with your child. If you need to scan and email your insurance card to victoria.lee@stratford.org, I can print them here.)
